Bereavement Services

High Country Health Care System Hospice provides bereavement services for up to 13 months to families of our patients and to anyone in the community who has experienced the loss of a loved one. Below is a list of some of the services that we offer in the Hospice Bereavement Program:

  • Listening and Encouragement
  • Home visits and/or telephone calls for a set period of time
  • Counseling in the home or scheduled appointments in the office regarding thoughts and feelings toward the natural process of death and dying
  • Weekly grief support group meetings at the Hospice offices
  • Face to face home or Hospice office visits within 6 weeks of the death of a loved one
  • Self paced grief process guided by quarterly mailings
  • Family Conferences
  • Birthday, Anniversary, and encouragement cards
  • Trained volunteers to visit and provide support
  • Spiritual guidance with the High Country Hospice Chaplain
  • Referral and connection to needed community services and resources
  • Optional services when requested including assistance in planning a funeral or memorial service and accompaniment to funeral home to plan services
  • Special sessions with children dealing with grief and the emotions of losing someone they love such as  Camp Sunshine

Our hope is that our Bereavement Program can be an extra support to you during the next year as you adjust to the loss of someone you love. We want you to know that you are not alone and we want to be here as extra support for you during this time. Please feel free to call the High Country Hospice office in your county and talk with the bereavement coordinator.
